1. Yes, we had to switch the in/out to the midi device. (IMHO- A small sacrifice for a small price.)
The USB side is connected to an old macbook.
2. It works just fine for what we needed. We've played the keyboard and had the sound generated on the computer at the same time.
We have no lag time with this configuration. Even the old computer running the latest version of Sebilius. There is no lag.
It was a very worth while inexpensive investment, for our purpose.

I was borrowing a friends Yamaha UX16 to interface my Alesis HR16 drum machine with my vista computer. I need to return that back to him so I bought this to see if it would work, at the price it was worth trying it out... and it worked great. I have since give it to my son so he could plug his new Micro Korg into his lap top, which worked perfectly and bought 2 more. The second one to hook up my Roland guitar synth to the computer. I was a little skeptical about this, thinking the price might be too low. The thing works beautifully with my Macbook Pro and solved all the midi-latency problems I'd been having with an M-Audio Firewire 410. One review on here I read stated that the MIDI out did not work. That was not my experience. I bought the device primarily for its MIDI out use, and it works perfectly, no latency. I plugged it in, it worked, immediately. Took me 2 seconds to plug this cable into my mac and keyboard after that garageband automatically recognized the midi and i was in business. I was making music after having the midi plugged in for 1 minute. Best midi for the price. Shipping was on time and convenient. I recommend this product to anyone who wants a great priced midi that works well. My midi add-on port for my Audigy 2z doesn't work with Windows 7 so I needed something to hook up my midi keyboard. I'm running Windows 7 64-bit and I didn't have to do a thing besides plug it in. The latency is good, I could tell no difference between using this and the old midi port. My only complaint is that the cord could have been a little longer, but a spare USB extension cord did the trick. This is a great product at a great value.
