The 3m Ergo mouse is the best mouse for ergonomics on the market. The picture is misleading though. I thought that perhaps it was some sort of joystick type of mouse (which would still require moving the wrist, which wouldn't help really). It is one solid piece that does NOT move at all. The picture also hides the fact that it has 3 buttons, a left click (on top for the thumb), a right click (also on top), and one on the side (not shown) for the other fingers.
What you'll immediately notice about this is that when you start to use it, your wrist will not move at all and you will be using your arm to move the mouse.
I have no problems with the mouse treating a single click as a double click either. Perhaps the other people simply had a defective unit (or more likely, 3M was producing it poorly has revised their design to have it fixed).
In my own experiment with this mouse, I intentionally did NO other therapy for my wrist to see how much the mouse actually helped. My hand went from nearly totally unusable to not having hardly any pain (unless I use a regular mouse again, and/or type for extended experiods again) in less than 20 days. Clearly, healing has already started, and it's also happened in less than 1 month with this mouse.
For people that play first person shooters, I also use this mouse for games with no problem at all. And it took me about 1-2 days to get used to it. It's very "natural". Highly recommended.

If your wrist is bothering you (or you don't want to risk that) and you are using a "normal" style mouse, you may wish to consider switching to this SPECIFIC mouse alternative! This specific "mouse" is better than the trackball type alternatives in that you don't have to keep flexing your thumb all the time. While using this mouse, your wrist and fingers all stay stationary - only your elbow is slightly changing to push it around. The weight of your hand is comfortably on the lower side of the hand which rests right on the mouse itself (so your hand never touches the desk). A right or left mouse click is performed by a very slight movement of the thumb up or down.
I got this mouse at work and immediately liked it so much that I bought one for myself at home too.
Switching to this mouse brought total relief to my wrist!
The only drawback is very minor - there is no scroll wheel - however there *is* a scroll wheel button that behaves like pressing down on the scroll wheel on a mouse. I use Mozilla/Firefox for my browser, and this "button" is fantastic to open a link into a new browser tab! The "button" is on the "handle" itself, so to "press" it you simply squeeze in your middle and fourth fingers slightly! Brilliant design.
I highly recommend this mouse (remember there is a LARGE and MEDIUM size to choose between though - and this is designed for use with your RIGHT hand)
Customer Review: Great mouse for "tennis/mouse elbow" Summary: 5 Stars
I am on my computer all day long at work and using a traditional mouse contributed to tennis elbow (the doctor called it "mouse elbow" since that is one of the common causes) in my right arm. My boss suggested I try to find an ergonomic mouse to help alleviate the problem. I tried both this 3M mouse and the Evoluent VM3R2-RSB Vertical Mouse. I have been using both for nearly a month now. I work from home much of the time and I have one in my home office and the other at work.
Both are great at maintaining a neutral hand position and I can now work for hours without additional arm/elbow pain.
This 3M mouse provides a more normal, comfortable hand and arm position for me - somewhat like a joystick at an angle. Unlike a joystick, it is fixed - it does not bend but slides on a desktop/mousepad like any optical mouse. The only think I miss is the scrolling wheel. It has a button that acts much as a scroll lock to allow movement to other areas of a worksheet but it seems cumbersome and I have not gotten used to it.
The Evoluent Vertical Mouse is very much like a standard mouse tipped on its side and does include a scrolling wheel. The drawback for me is that the angle is a bit too vertical to feel natural or neutral for my hand and arm.
Both the 3M and the Evoluent mouse definitely make long-term mousing more comfortable and I can recommend them both.
Customer Review: Perfect for those with wrist pain Summary: 5 Stars
This 3M Ergonomic Renaissance Mouse is great for those with wrist injuries or discomfort because it keeps your thumb pointed in the upward direction, eliminating pressure within the carpel tunnel area. I was pleased to find this new version that uses an optical sensor, a USB connector, and a new graphite color. Mouse model number EM500GPS is supposed to be for those with palms 3.5 inches wide and less and EM500GPL is for those with palms 3.5 inches and larger. My palm is exactly 3.5 inches wide, I tried both, and the large size was much more comfortable. My fingers were much too cramped with the smaller mouse. I think the recommended cut-off between the two sizes should be about 3.0 inches. Unfortunately, there are a couple of drawbacks. First, it's for right-handers only. Secondly, it is not quite as precise as a regular mouse because you can't use your fingers to fine tune the position of the pointer, so I occasionally miss target areas and have to reposition the mouse and click again. However, I still highly recommend this product because it helped eliminate my wrist pain, and that is the first priority! I have been wearing wrist splints for Carpal Tunnel periodically since 1998 and constantly for the past three months (only getting worse). I work in a mouse heavy usage environment, 40+ hours a week. I got the mouse last night and took it into work today. It was truely plug and play. It came with a USB connection and an adaptor for regular use. I love the fact that it is an OPTICAL mouse which allows precision pointing and ease of use. There was no learning curve at all! Today is the first time in 3 months I have not had to use ice packs and Aspercreme in addition to ibuprofen to make it though the day!!! I wish I had known about this mouse 7 years ago when I began having problems. My hand felt weird at first, only because I was actually experiencing normal sensations in my hand instead of pins and needle numbness. This mouse is a true blessing that will allow me to continue working until I have my release surgery. If you have problems with carpal tunnel or "tendonitis" I HIGHLY suggest you buy this mouse!!! Catch it early and avoid surgery and weeks of loss time at work.
